A Feedback button for your site with two ways in. Visitors can talk it through with a voice agent that asks short follow-up questions until their bug or suggestion is clear, or type it in the written form. Either way, you read feedback you can act on.
Two ways to give feedback
Talking is an option beside the written form, not a replacement for it. Visitors pick whichever suits them, and can switch at any time.
Voice
A short conversation
The agent asks one question at a time: what they want or what went wrong, where, and why it matters. When they press Finish, it writes a summary in their own words, ready to review, edit and submit.
The audio goes to OpenAI to run the conversation. Only the written feedback the visitor sends is kept, never a recording.
Form
The written form
A bug or a suggestion, in a few words, without leaving the page. An email is optional, for when they would like a reply.
Nothing to allow and nothing to install: it works on any page that runs the widget.

Clearer than a blank box
A visitor who would write "it's broken" is asked what they did and what they expected, so the message says what happened.
One script tag
No SDK and no build step. The widget is a few kilobytes and keeps its styles to itself, so your page stays as it is.
Enough to act on
Each message keeps the page it was sent from and, when the user leaves one, an email to reply to.
